prospective puppy/adult buyers. . . . . . . .
We ask anyone interested in purchasing one of our puppies or adults to complete our buyer questionnaire. Please copy and paste the following questionnaire into an email, or print and fill it out and email to us at Debbe@SundanceAussies.com or mail it to the above address.

23. Are you willing to enroll the puppy in at least one structured obedience class?
24. Where will the puppy/dog be when no one is at home?

29. If you are interested in a pet/companion, will you enter into a pet contract (spay/neuter certificate must be received by Debbe Dolson - Sundance Australian Shepherds, before the age of 9mos)?
30. If you are interested in a show/breeding Aussie, will you enter into a show/breeding contract (this means that the puppy/dog must finish it’s AKC & ASCA championship and will remain in a co-ownership with Debbe Dolson - Sundance Australian Shepherds until this is accomplished or the puppy’s/dog’s individual registration papers will be marked “not for breeding purposes” and at which time the dog has accomplished the above, I, Debbe Dolson, the litter owner, will change the papers with the appropriate registries)?
31. Are you willing to have the puppy’s/dog’s eyes checked annually by a licensed veterinarian?
32. If your plans are to one day breed your puppy, are you willing to have the puppy’s hips x-rayed and submitted to either PennHip or OFA after the puppy is 24 months of age and before it is bred?
33. If you will be doing agility, are you willing to have the puppy’s hips x-rayed or OFA’d before starting to compete in agility?
34. Are you aware of the health problems which can appear in some Aussies, such as medication sensitivities?
35. Are you willing to participate in testing your puppy/dog for health/genetic research purposes if needed?

37. Please give us two references (name and phone number) of people that know or have known you and your dogs/pets.

Thank you very much for taking the time to fill out this questionnaire. The purpose behind these questions is that we have observed the puppies from the beginning and may be better suited to evaluate them for your needs and goals and help to pick the puppy you’ve dreamed about!
